Paperwork for the hospital

Be sure to bring your child's Montreal Children's Hospital card, Medicare card or details of your health insurance plan. You should also bring your child's immunization record and any referral or pertinent medical information from your child's doctor.

Please also have the name, address, telephone number and fax number of your child's doctor.
